Confidential Juror Questionnaire

Massachusetts law requires that potential jurors complete a Confidential Juror Questionnaire (CJQ), for use during the impaneling of a jury. This information is kept confidential, and is not used outside the courtroom. Virtually all CJQs are destroyed the same day the potential juror appears for jury service.

The CJQ has three sections and takes about five minutes to complete. Click this Get Questionnaire link to open the CJQ form online. You will be able to complete the CJQ on your screen and then print it. Bring the completed CJQ with you to the courthouse on your day of service. You will also receive a blank CJQ in the mail about two weeks prior to your service.

No information that you complete on the online CJQ will be saved or transmitted, so as not to put your personal information at risk. You must PRINT the CJQ and bring it with you to the courthouse.
